Our CLS program is a package of health-related and supportive services available for seniors or individuals living in a congregate, a group home, and other known residential facilities. We also collaborate with home support services, helping people with developmental disabilities build strong connections with the community, get assistance with home safety planning, and other services needed.
Our Customized Living Support program includes:
